JCHS EARLY RELEASE FORM

From time to time, students have valid reasons to leave school grounds before the end of the regular school day. In order to safeguard their wellbeing, a strict protocol is in place. To start the process, a parent or guardian must request an early release through this form at least 1 hour prior to the time of dismissal, with the option of submitting it up to five days in advance. 
Please note: authorization must be given separately for each child requiring early dismissal. This is necessary to ensure each student's request is accurate and accessible.

SCHOOL LUNCH FORM

Please use this form to order and pay for your child's lunch each week. 

You can order for the entire week ahead or day by day. Each days order will need to be submitted by 7PM the night before. Any orders submitted after 7pm (for the next day) will not be accepted and/or delivered to your child. We advise getting all orders submitted in the beginning of every week. 

All lunch meals should be ordered directly on this form except for CATCH. WEDNESDAY orders from CATCH will have it's own separate lunch form. 

If you are ordering for Siblings, please make 2 separate orders. The students in different grades, do not always eat lunch together. This will make it easier for us to make sure each student receives their correct lunch in the correct location.
